	 Do you have to repeatedly tell your teenage son to turn down the volume of his iPod just so he can hear you? Ever listen to him complaining of feeling ringing, buzzing or hissing noises in his ears? These are the two most evident symptoms of hearing loss.


	 Recent studies showed that today two in every five teens have hearing loss in some form or another, which is 30 per cent higher than it was in the 80s and 90s. Now most iPods and mp3 players can generate sounds that measure up to 120 decibels, equivalent to the sound produced at a rock concert. The hearing loss caused owing to the use of headphones is extremely gradual, with no clear cautionary signs; a hearing examination is the only way one can diagnose hearing damage.


	 Common Symptoms

	 How does the ear get damaged?

</h2>

	 The innermost area of the ear has nerve endings in the form of small hair cells. These nerve endings change the sound waves into electric waves. The nerves then carry these to the brain, which identifies the sounds. These minute nerve endings may be damaged on exposure to sounds that are too loud.

<h2 style="text-align: justify;">

	 When are you at risk?

</h2>
<ul>
	<li>High volume of the music – there is risk of hearing loss in exposure to sounds 100dB and above over periods of two continuous hours or less per day, with chances of suddeninstantaneous loss at exposure of 120dB and above.</li>
</ul>
<ul>
	<li>When you are sitting too close to the speakers.</li>
</ul>
<ul>
	<li>Listening to loud music frequently and using earphones.</li>
	<li>History of hearing loss in the family indicates a sensitivity of the ear, so it’s better to be careful in such cases.</li>
	<li>
	
		 Occupational exposure to loud sounds, as in being a sound/audio crew-member or a musician, working in nightclubs or being exposed to artillery firing etc.

	 Go Retro With Earphones


	 While listening to music with personal music devices like mp3 players, preferably use noise cancellation headphones. While using the ear bud type headphones, users tend to increase the volume to cut down on the surrounding noises. Headphones that go over your ears can also damage your hearing if you use them too long or play music too loudly. They are just not as much of a risk as ear insertion types are. Having the source of the sound in your ear canal can increase the sound’s volume by 6 to 9 decibels – enough to cause some serious problems. If you listen to music at high volume (level five or more) for 15 minutes each day, it can damage your hearing in the long run. So keep the volume low.


	 Diagnosis


	 After examining the ears physically, your doctor will order a few tests. The basic test is pure tone audiometry, which typically shows a sensorineural hearing loss predominantly at 4 kHz, and impedance audiometry or MRI of the brain and cerebellopontine angle, if any other cause of hearing loss is suspected.


	 Treatment


	 The treatment depends on the cause. Generally, a short burst of steroids are tried for sudden onset of hearing loss with nerve supplements like high doses of vitamin B6 and B12, with gingko biloba extracts and peripheral vasodilators.

	 Prevention


	 There is no situation where the old adage ‘prevention is better than cure’ is more applicable. When at a concert, the farther you sit from the speakers the better it is (at least 10 feet away). Move to quieter areas in between to give rest to your ears.


	 In all that noise, others might shout into your ears to be heard – avoid that, as it damages your ears more. After any kind of noise exposure, give your ears at least a day’s rest.

Balloon Sinuplasty - Less Invasive Quicker Recovery

	 One can have ear infections at any age. Earache and feeling ill are the first symptoms. Antibiotics are given only in a few cases, whereas painkillers are prescribed as the main treatment. Usually the infection clears in a few days.


	 The two main types of ear infections are - those that occur in the outer ear canal (otitis externa) and those that occur in the tiny space behind the eardrum (middle ear).


	 Even though earache is a common symptom of ear infection, all earaches are not infections. A common cause of mild earache is a build-up of mucous in the middle ear after a cold, which usually clears in a few days. Sometimes, the pain that is felt in the ear is from other causes like jaw joint pain or tooth problems (referred otalgia).


	 Swimmers Ear (Otitis Externa)


	 This condition can crop up when bacteria nestle into an outer ear canal that is warm and moist. Being in the water a lot also tends to wash away the natural oily, waxy substance that normally lines and protects the ear canal. Bacteria can then get the upper hand and you get an infection. Water can be left in your ear after taking a shower, or poking around with a bobby pin or cotton-tipped swab can scratch the delicate skin in the ear canal and break down the barrier against bacteria, resulting in an outer ear canal infection also called otitis externa. Resist the urge to scratch, as that will make the problem worse. Don’t let the water sit in your ear. Shake the water out after a shower or swim. The best way to dry the ear is to roll the corner of a soft cotton handkerchief and use it to mop the canal.


	 Middle Ear Infection (Otitis Media)


	 Middle-ear infection (known medically as otitis media) is the most common illness in babies and young children. The outer ear is connected to an aircontaining space called the middle ear. The air pressure in the middle ear is equalised more than one thousand times a day every time you swallow, through the Eustachian tube, which also carries fluid away from the middle ear.


	 When a cold or an allergy is present, the Eustachian tube swells and air is absorbed by the lining of the middle ear, creating a partial vacuum, and fluid weeps from the lining of the middle ear. Bacteria or viruses infect the stagnant, warm fluid in the middle ear. Children may be more prone to middle-ear infections as their Eustachian tubes are shorter and straighter than those in adults; they get colds and sore throats more often than adults as their immune system is not fully developed. Most bouts of ear infection will clear within two or three days, on their own without treatment.


	 Antibiotics are mostly prescribed in the following cases:

	 Prevention

<ul>
	
		 1. Keep your child away from other kids who have infections.
	
	
		 2. Take care of nasal allergies and teach your child to blow the nose gently.
	
	
		 3. Do not expose your child to cigarette smoke.
	
	
		 4. Avoid giving a bottle of milk or formula to a baby who is lying on his/her back, because the nutrient-rich liquid can flow into the Eustachian tube during swallowing and pool there, creating a breeding ground for infectious organisms. Prop the child’s head up on pillows.
	
	
		 5. Occasionally, some children have recurring bouts of ear infections close together. If this occurs, the ENT specialist may advise a long course of antibiotics to prevent further bouts from occurring.

	 At times, the ENT specialist may advise the insertion of a grommet or tympanostomy tube into the eardrum if ear infections are very frequent. A grommet is like a tiny pipe that helps to drain fluid out from the middle ear and allow proper ventilation of the middle ear.

	 Chronic sinusitis or rhinosinusitis is a condition that affects almost 134 million people in India/which means one in eight do not have a healthy sinus. The number of affected persons is more in the metros like Bangalore, Mumbai and Delhi thanks to the higher levels of pollution and lesser green spaces and fresh air. Everyone has four types of sinuses which are hollow cavities in the bones around the cheek bones and eyes. Each sinus has an opening, through which accumulated mucus can drain. When this drainage is blocked, due to allergic reactions, microbial infections etc, it results in sinusitis. The function of the normal sinuses is to help in cleaning up the air one breathes, much like the filter in a carburetor of an automobile.


	 The most prevalent surgical procedure used to open blocked sinuses is functional endoscopic sinus surgery (FESS). However, al-though FESS is able to clear blocked sinuses it may not necessarily return normal functioning to the sinuses. The removal of the bone and tissue also leads to scar-ring, bleeding and post-operative discomfort to the patient.


	 Balloon sinuplasty is the fruit of combining endoscopic technique with emerging technology and clinical research proves the balloon sinuplasty method of surgically addressing sinusitis is a proven, minimally invasive option to dilate blocked sinuses.


	 The technique uses a small, flexible, sinus balloon catheter, which is placed into the nose to reach the sinuses. This is gradually inflated to gently restructure the previously-blocked sinus passage, maintaining the integrity of the sinus lining and restoring nor-mal sinus drainage and function quite similar to a cardiac balloon angioplasty. Its advantages over conventional surgery are many being much less riskier than other surgical methods with no adverse events being reported. Patients received significant relief from their symptoms at multiyear follow-ups.


	 This method is less invasive, with less pain, less recovery time (just 24 hours for most patients), less scarring, less bleeding and less follow-up procedures. It can be used safely in tandem with other medical therapies or sinus surgery techniques. It does not limit future treatment options, and is safer for people who are prone to excessive bleeding or those in the ICU.

Balloon Sinuplasty - Less invasive, Quicker recovery

Anyone who’s seen a nosebleed will vouch for the fact that it can be a scary sight. Usually, there will be blood oozing from one or both nostrils, and the incident will be referred to as epistaxis medically speaking.


	 Although it looks alarming, most of the time the nosebleed will stop on its own. All it takes is a few minutes of self-care. The intensity and the time of the bleed could differ from person to person, but most nosebleed episodes last between a few seconds to a few minutes.


	 Medical attention will be required only in cases that are heavy and/or frequent require medical attention.


	 Most often, nosebleeds are caused by the mucus lining inside the nose drying up - which could happen if you live in a cold, dry place; spend lots of time in air-conditioned areas; or due to an infection. Nosebleeds could also be caused by deficiencies, inhalation of dangerous chemicals, sinusitis and substance abuse. The usual routine for stopping the bleed is to hold/pinch the area above your nostrils for ten minutes.


	 People whose blood takes longer time to clot, whether because of their body constitution, or due to some medication they are taking- also suffer from longer and heavier bleeds. In cases where ‘pinching of the nose’ routine does not work, and the episode lasts for more than twenty minutes, then the doctor will have to try other options to solve it. Cauterising the bleeding point is one option - the bleeding point is sealed with either chemicals like trichloroacetic acid or silver nitrate or with electrocautery. If the bleeding is very profuse, the nose is tightly packed with ribbon gauze soaked in antibiotic creams or with special hemosealent sponges.


	 For an episode that lasts more than twenty minutes, it is very important that you get professional help to check why this is happening. Your doctor is likely prescribe a few blood tests like a complete hemogram to see is the patient is anemic or whether he/she has an abnormal bleeding tendency due to blood cancers like leukemia, lymphoma , idiopathic thrombocytopenic purpura etc. Sometimes acute nosebleeds could be the sign of certain kidney ailments like acute nephritis, chronic renal failure etc and to check this out blood urea levels, creatinine levels and blood uric acid levels are checked. Liver failure or some variants of hepatitis also can cause a nose bleed, so a full liver function tests also, including a check of bleeding and clotting parameters.


	 People who suffer from hypertension or high blood pressure are likely to have heavier bleeds that are usually posterior in nature. This means that the bleeding is not happening from the front of the nose, but it is happening from the top. For BP patients, this can be a chronic and sometimes frequent experience. Chronic bleeds require medical attention, since this heavy blood loss can be serious and in some cases could even lead to anemia.


	 People who have nose bleeds due o high BP should monitor their blood pressure carefully and ensure that it stays within the healthy limits. This can be done by following a healthy food pattern that helps to regulate blood pressure, proper exercise, getting adequate sleep, and finding a good way to destress.


	 If the nose bleed is not profuse and only blood streaked mucous generally it could be due to simple causes like dryness, finger picking of the nose or acute rhinosinusitis. On the other hand, having persistent blood streaked mucous could be a symptom of something more serious like cancerous lesions of nose and nasopharynx or fungal infections. If these are not identified and treated early, it could become life threatening. It is very important that these be ruled out by an ENT specialist.


	 If you are generally prone to nosebleeds, try to avoid picking your nose, or blowing too hard through your nostrils. Be careful about the blood-thinning medicines that you use, and do discuss with your physician or specialist if you are on such medications. It is also advisable to avoid alcohol, smoking and hot drinks, especially when you are recovering from a nosebleed, since these will make the blood vessels inside your nose to expand.

Have you been having trouble every time you catch a cold? What starts off as a simple sniffle turns into a few weeks of discomfort or even debilitating sinusitis? Chronic sinusitis, or the incidence of having heavy blocks in your nasal passageways, affects more than 37 million people in the USA annually. With the rise in the incidence of allergies and pollution, urban India is also facing a huge rise in this condition.


	 So what exactly is sinusitis? We have four types of sinuses on each side of the face- hollow cavities around the cheek bones and eyes. These cavities are covered with a mucus layer and cells that contain a lining of cilia or tiny hair, which trap and push out the bacteria and pollutants that enter the cavities. Each sinus has an opening, which allows accumulated mucus to drain out. When this drainage is blocked, that’s when a mucus buildup starts to occur. Due to allergies etc. many people suffer from improper drainage, microbial infections etc. which causes accumulated mucus and/or inflammation of the membrane in their sinuses. These cases result in chronic sinusitis or as acute sinusitis.


	 Debilitating condition


	 Sinusistis, especially if it’s an acute case, can be a very debilitating condition. Pains and aches around all of the cavities, intense headaches, fatigue, congestion in the nasal passages and sinuses, loss of smell, heavy headedness – separately, one may be able to handle them, but when they come in together, these effects can be totally debilitating. Statistics say that chronic/ acute sinusitis is actually more debilitating than congestive heart failure or chronic back pain! US statistics say it results in 18-22 million doctor visits every year.


	 ENT specialists call this condition Rhinosinusistis, which could be classified as acute (when the episode lasts for less than 4 weeks) or as chronic (when the episode lasts for more than 12 weeks).


	 Surgical Consult


	 Statistics from the US show that there are 1.4 million surgical consults for chronic rhinosinusitis annually. Of these, 900,000 decline surgery – either because they are ineligible for the Functional Endoscopic Sinus Surgery (FESS) or because they do not feel comfortable about doing it.


	 In any sinus surgery, what the ENT surgeon focuses on will be to clear the blocked sinuses, and restore normal drainage and function to the sinuses. Amazing doctor! Best ENT doctor in Bangalore. I met Dr. Anita in 2012 during sudden hearing loss on my left ear. Based on her advice, I was admitted to hospital and recovered my complete hearing.
